Clients can contact their insurance companies to find out if they have out-of-network benefits with their particular plan. I have self-pay, fee-for-service clients as well. I accept payment with checks, cash, debit and credit cards.
Prior to Your First Appointment:
Please contact your insurance company to confirm the following information: if I am an in-network provider for your plan; if I am not an in-network provider, do you have out-of-network benefits with this plan, and, if so, how does that coverage compare to using another in-network provider; the amount of your deductible and what has been met to-date; what is the copayment or coinsurance, the amount you are responsible for after the deductible has been met; and whether or not pre-authorization is required. Please bring your insurance card to your initial appointment. If the primary insurance holder is someone other than yourself, insurance requires you have their date of birth for billing purposes. I bill insurance directly and you are financially responsible for any uncovered expenses.
